MyChart is an online tool that provides secure access to portions of your medical record at Southside Community Health Services. It conveniently helps you manage and receive information about your health.

Southside Community Health Services accepts self-pay, private insurance, Medicare, and MNsure products including Medical Assistance, Minnesota Care, and Qualified Health Plans (QHP). Southside has a Sliding Fee Discount Program for those who quality. No one is ever turned away due to lack of insurance or inability to pay.
